This one is a little newer and even worse:

Even with a cage on top of the lift, all that material falling has a LOT of weight pushing debris in from each side. That's gotta hurt.

Truthfully, there were no less that 1,477 video cameras on site when that happened.
But none of the rats knew how to operate them.

The real reason for no video was that there were no rats to operate the cameras. When sh-t is about to hit the fan they know to scatter and come back later for easy pickins.
