What is this and was It the problem? It looks like an insulated tie bar for operating the switches and it looks broken.

That ties the movable contacts to the CBC mechanism, and yes it is broken in the IR picture you can see a thermal issue as the contacts are not properly closed.

The damage caused by operator error ??

By not following the correct prodcure to order to disconnect the device in correct format.

Most issues I see with bolted pressure switches are operator error. In this case I believe this is a defect with the cross arm. The customer tried to open the switch and it locked up.

Just from a quick look the resin bar seems light for the amount of leverage it needs, the point of attachment for the arm should have been beefier.

RK: I have serviced literally 1000's of these switches and installed 100's, this was the first time I have seen this happen. What you can't see is the metal plate across the top where the operator arm attaches to the cross arm.
