    Delta wye transformers that are paralled on the secondary side can cause problems. The secondary breaker of one of the transformers failed to open from reverse current; however the source breaker (12 KV) opened up due to a fault and now you have an unaffected grounded system which put 12 KV across 7.2 kv phase to ground transformers primary and the seondary voltages were elevated to a much higher voltage than 208V and caused TV sets to explode. Was a big lawsuit.
