Default Old Boss Still Hasn't Sent My 2017 Hours To The State of Michigan

My old boss keeps dodging me when I ask him if he could please send my hours to the state of Michigan. It's over 2000 hours that he needs to send.

Do I have any legal action?

call the employment relations commission (i think that's it) they deal with this type of stuff, they'll call him and he'll probably have it taken care of the next day.
