How could this go on so long the company folded because of it.

Sounds crazy to me too. I can't imagine how they got away with it for so long.

One reason was the owner hung his shingle too soon and wouldn't listen to his supervisors. Also, he kept forgiving the employees in question.

He was too busy doing hands-on and not enough incremental supervision.
