Sounds like someone brought in two branch circuit hots (from different poles in a 240 volt 1 phase panel) and tied one to the ground screw and the other to the neutral.

Or the OP info is wrong.

My first thought was, is It a 240 V AC outlet at one time, but someone would have to totally miswire the connections.

Sounds like a huge miswire. Someone took neutral and ground on a double pole breaker.
