Two family home. Detached garage with two garage doors, a man door for each side, and a wall separating the two sides. Each tenant has access to only one side.

Would you be allowed a MWBC for each side?

It's not a riddle. I ran into this once on a side job (a long time ago) and didn't really care if it was legal to do or not at the time.

There's this...

225.30 Number of Supplies. A building or other structure
that is served by a branch circuit or feeder on the load side
of a service disconnecting means shall be supplied by only
one feeder or branch circuit unless permitted in 225.30(A)
through (E). For the purpose of this section, a multiwire
branch circuit shall be considered a single circuit.
Where a branch circuit or feeder originates in these
additional buildings or other structures, only one feeder or
branch circuit shall be permitted to supply power back to
the original building or structure, unless permitted in
225.30(A) through (E).
For the purpose of this section, a multiwire branch circuit
shall be considered a single circuit.
(B) Special Occupancies. By special permission, additional
feeders or branch circuits shall be permitted for either
of the following:
(1) Multiple-occupancy buildings where there is no space
available for supply equipment accessible to all occupants
Who gives the permission? Did the NEC just give the permission? or do I have to have the inspector's blessing?

Who gives the permission?
The AHJ which may or may not be the inspector you deal with.

Special Permission. The written consent of the authority
having jurisdiction.

Authority Having Jurisdiction (AHJ). An organization,
office, or individual responsible for enforcing the requirements
of a code or standard, or for approving equipment,
materials, an installation, or a procedure.

Since there is no common area in the garage...

I believe if there is a fire wall to the roof between the two sides then they may be considered two structures

Ive seen this before in Condos, and the looks of it was yes, its ok. But code might allow it to since its owned and metered by 2 different tenants.
