Square D QO breaker washers twist off wire

Anyone else have problems with the Square D QO breaker washer twisting inside the breaker on the wire? Is there a solution for this? Picture below.

Looks like operator error to me.

Square D can do no wrong.

washers tend to spin most times when you tighten a screw or bolt...

pretty easy just hold the washer with another screwdriver on final torquing of the screw. also the wire should be completely hidden under the washer

I would think it makes a good connection , even though the washer is twisted .
