Installing some doors, and the manufacturer want this cable bent upwards to allow for a side cover to be installed, the cover has less than 1.5" clear from the steel end plate to the inside face of the cover. We will need to accommodate a 90 degree bend within this 1-1/2" space.

The cable is 600V 4 wire 12 gauge with drain shield. I used the search on the site and found that the (5D) bending radius only applied to over 600V.

Is there a problem with this?

Thank you

Meh, it'll be fine for the life of the equipment. Nothing I would worry about.
