Very hot connection... The heat is at the conductor to connector most likely. Note the fish paper had thermal damage. I have never tried to burrn fish paper, but have seen it severly damaged from heat, but not burning...HMMMMMMMM next test.

Wow-wee! The lug looks like the set screw isn't even is as far as the connection right above it. I'm always suspicious when I see a series of mechanical lugs, and one of the set screws is obviously torqued in further than the others or not as far as the others.

That's an interesting fuseholder, too. Looks like bolted connection of some sort right to the fuse blade.
