Hello... Installing an electric hot water heater... 220... I speak 110 pretty well, but 220...aaa not so much.

Heater has only to hot lines coming out and i am curious if i need a neutral at all? Client says yes just run it to the heater and attach to ground.

You are construction related but not specifically an electrician. You have asked a specific question, but there is a good chance that the owner has done other things wrong that you have missed.
An electrician would tell you that connecting a true neutral (grounded) wire from the panel to a ground terminal at the heater is totally wrong, against code, and unsafe.
What else may be wrong that the inspector will find, or worse, that the inspector will not find?
