The Leviton 5059 is close, but as an assembly not usable unless modified.

I have a Wiremold 377B box in my office with no receptacle, but would hold the same device.

No EGC anyway. Grab your chisel and hammer.

 

Agreed on the egc but there's a chance we can pull new wire in, I've done it in worse looking boxes and pipe. I've debated cutting a new floor box in, the only issue is the pipe is only about 2-2.5" below the floor. Not a major problem though, just a PIA!!

Put a regular cover on it.

 

I often use regular covers when there isn't a traffic issue. Looks a lot better than a true floor cover. This is residential only and in places like under a handrail in a loft or under a end table next to the couch.

Hmm, I'll have to see if our suppliers have anything in any of there branches. Screw holes might be to narrow but there's a chance!

Pretty sure the receptacle alone is Pass and Seymour 5251SS**B available Ivory(I), White(W), Black(BK), and Light Almond(LA)

Walkerduct (Wiremold// Legrand) trimmings?

Your picture shows what appears to be 2.5" presets... or the grandfather thereof.

436- 2-2 3/8 2 3/8" [64mm] high

This is the Walkerduct number for the pre-set in your photograph -- though yours looks considerably older, and is the basis for # 436-2-2 3/8.

You want an "ACTIVATION PACKAGE" for for this preset.
